Henry Schein One recently launched Forms, a digital patient intake solution now natively available within the Dentrix and Dentrix Ascend practice management platforms. The feature is designed to minimize manual data entry and reduce reliance on third-party tools by integrating digital forms directly into the patient workflow.
According to the company, more than 1,000 practices have adopted Forms since its soft rollout earlier this year. Initial internal data from early adopters suggest that 90% of patients complete their forms once started—a potential time-saver for front-office staff and an opportunity for improved visit preparedness.
Forms includes a built-in form builder with drag-and-drop templates for creating customized forms based on appointment type or visit status, as well as multilingual form options to accommodate diverse patient populations. The platform currently supports scanning patient IDs and insurance cards to autopopulate form fields, and credit card scanning is planned in future updates. Forms automatically integrates patient responses into the chart and the solution also offers form status tracking, digital signature functionality, and automated reminders.
By embedding these capabilities directly within Dentrix and Dentrix Ascend, Forms allows practice staff to monitor intake progress without logging into separate platforms or re-entering data manually.

"Patients want ease, staff want time back, and practices want accuracy. Our new Forms solution delivers all three."
Michael Wilson, vice president of IT systems & infrastructure at Today’s Dental Network, highlighted customization as a primary advantage. “Our doctors take a lot of pride in their patient correspondences, and they’re now able to customize them with pictures and logos,” he said. He also noted that better information density on individual forms has reduced the volume of paperwork sent to patients.
For dental service organizations (DSOs) and group practices using Dentrix Ascend, Forms offers centralized management of form templates. Administrators can create and deploy forms across multiple locations, allowing for a consistent patient experience while maintaining flexibility at the local level.
Forms is included at no additional cost as a part of the standard Dentrix Ascend upgrade, as well as within Patient Engage or Lighthouse. Dentrix users not currently using those services can subscribe to Forms for a separate monthly fee.
